• Name of the person receiving the gift voucher 

• Consultation / Treatment / Kur that you would like to gift 

• Your queries if any 

We could then make an invoice for the amount and send it to you by e mail.

After you transfer the amount to our bank account, please send us an update.

We will send you the gift voucher as PDF.

The person receiving the gift voucher can then contact the Praxis and make a suitable appointment.
